Position Summary:
Reporting to the Medical Services Manager, the Intake Worker provides confidential, trauma-informed, and culturally respectful intake for community members seeking mental health and addictions support. The role assesses needs, strengths, risks, and goals; coordinates referrals and follow-up; supports service navigation; maintains accurate records; and collaborates with Wellness staff, health providers, and community partners to improve timely access to care.

Qualifications:
• High school diploma or equivalent with relevant frontline experience; related post-secondary education in mental health and addictions, social services, community services, Indigenous wellness, counselling, or a related field is preferred.
• Knowledge of intake, referrals, consent, confidentiality, documentation, crisis response, trauma-informed practice, harm reduction, recovery, aftercare, and community wellness resources.
• Experience with First Nation communities or Indigenous, health, wellness, crisis, addictions, or community-based programs is a strong asset; Indigenous language skills are an asset.
• Valid Ontario Class G driver’s licence and reliable vehicle when travel is required
• Criminal record and vulnerable sector checks;
• First Aid/CPR, ASIST, safeTALK, Mental Health First Aid, Nonviolent Crisis Intervention, harm reduction, trauma-informed care, or related training is an asset or may be required.
• Flexibility for occasional evenings, weekends, or extended hours.

Duties and Responsibilities
• Serve as a welcoming and confidential first point of contact for community members seeking mental health, addictions, crisis, aftercare, or wellness-related support.
• Complete initial intake conversations by gathering information about each community member’s needs, strengths, risks, supports, consent, and service goals
• Recognize urgent concerns and follow crisis-response, safety-planning, mandated-reporting, and emergency-referral procedures.
• Connect community members with appropriate internal and external services and clearly explain referral requirements and next steps.
• Coordinate referrals, follow up on appointments, advocate for community members, and support continuity of care
• Maintain accurate, timely, and confidential intake, consent, referral, follow-up, and service records.
• Provide trauma-informed, culturally safe, strengths-based support that respects Indigenous traditions, community protocols, and approaches to healing
