Nurse Practitioner - Nurse Led Outreach Team

Position Summary: The Nurse Practitioner (RN(EC)) for the Nurse Led Outreach Team (NLOT) program uses advanced practice clinical skills to provide direct clinical care, education, research, consultation and treatment around Geriatrics to support the acute care needs of residents in LTC. This role is central to improving access to timely, high-quality medical care for older adults living in LTC, reducing avoidable hospital transfers, and supporting a more coordinated and resident-centered approach. 

Position Responsibilities:  

  • Conduct comprehensive health assessments, diagnose, and manage acute illnesses, and provide timely follow up care within LTC homes.
  • Prescribe medications, order and interpret diagnostic tests, and coordinate with attending physicians and LTC staff.
  • Select appropriate interventions from a range of non-pharmacological and pharmacological to restore or maintain resident function, physiological, and mental stability to achieve optimal health.
  • Communicates with residents about health assessment findings and/or diagnosis, including outcomes and prognosis.
  • Document assessments, intervention plans, progress notes, and outcomes in a timely manner.
  • Support advanced care planning, goals of care discussions, and palliative care delivery in alignment with resident wishes.
  • Serve as a liaison between LTC homes, hospitals, specialists, and community partners to ensure continuity of care.
  • Provide clinical leadership, mentorship, and support to LTC nursing staff to enhance capacity and care quality.
  • Participates in case conferences, family meetings, and quality improvement initiatives.
  • Capture relevant and required data as defined by Ontario Health, Health Quality Ontario, Accreditation or other quality improvement strategies.
  • Align improvement projects with the organization and monitor or adjust to achieve greater outcomes.
  • Evaluate services and satisfaction on an on-going basis and formulate improvement strategies and solutions.
  • Perform other duties as required.

 

Position Requirements:  

  • Registered Nurse Practitioner in the Extended Class.
  • Master’s degree in nursing (MScN or MN) is an asset.
  • Registered and in good standing with respective regulating body.
  • Minimum of 3 years’ experience working with older adults.
  • Experience in program development and education.
  • Demonstrated knowledge of evidence-based practices.
  • Effective time management and ability to prioritize workload.
  • Must possess the ability to work with minimal supervision and must follow standardized procedures.
  • It should be adaptable to cope with changing contacts with people, situations and conditions.
  • Must have an employment history which demonstrates regular attendance at work.
  • Must possess the ability to provide effective services to clients/patients in both English and French. Individuals who do not meet the language requirements of this post are invited to submit their résumé for consideration.

  • Must have had at least one doses of the COVID vaccine.
  • Starting salary of $60.99/hour; $118,930.50/year (As per Step 1 of the ONA collective agreement.)
  • Option to enroll in Healthcare of Ontario Pension Plan.
