Wellness Coordinator

                                                Interim Position

Position Summary:

The Aftercare Addictions Coordinator supports individuals and families transitioning from addictions treatment back into their community. This role ensures culturally appropriate, client-centered aftercare services grounded in Indigenous values in recovery, traditional healing, and community-based supports. The coordinator works to promote long-term recovery, wellness, and connection to culture, land, and community.

 

Key Responsibilities:

  • Develop individualized, culturally safe aftercare plans in collaboration with clients, families, Elders, and the treatment team.
  • Coordinate follow-up care including counseling, cultural supports, peer recovery programs, and access to mental health, housing, employment, or education resources.
  • Facilitate access to traditional healing practices (e.g., sweat lodge, smudging, land-based healing, ceremonies) in alignment with client goals.
  • Conduct regular wellness check-ins through home visits, phone calls, or community gatherings.
  • Support reintegration efforts by connecting clients with community programs such as parenting support, employment readiness, and harm reduction services.
  • Advocate for client needs and work to reduce stigma associated with addiction and recovery.
  • Maintain accurate and confidential records, ensuring alignment with community/Social Development Department protocols and privacy standards.
  • Support community education and awareness around addictions, mental health, and wellness.

 

Qualifications:

  • Diploma or degree in Social Work, Addictions, Indigenous Wellness and Addictions Prevention, or a related field
  • Minimum 1–2 years of experience in addictions recovery, aftercare planning, or Indigenous community health.
  • Knowledge and respect for Indigenous history, culture, traditions, and the impacts of intergenerational trauma.
  • Familiarity with the Medicine Wheel, holistic healing models, and Indigenous determinants of health.
  • Ability to build trusting relationships with clients, families, and community partners.
  • Strong communication, case management, and advocacy skills.
  • Ability to work independently and as part of a collaborative team.
  • Basic computer skills and knowledge with Microsoft Office.
  • Valid G Drivers License.
