Nord-Aski Family Health Team is seeking a dynamic, organized, and reliable individual to fill the permanent full-time position of Registered Practical Nurse. This position is part of a collaborative practice model between the Nord-Aski Family Health Team and the Hearst Notre-Dame Hospital Locum Clinic, aimed at improving access to care and continuity of services for the community.

Working under the supervision of the Director of Patient Services of Notre-Dame Hospital and the Executive Director of the Nord-Aski Family Health Team, the successful candidate will provide high-quality nursing care to patients within their scope of practice. They will work closely with the interdisciplinary team (physicians, nurse practitioners, and other healthcare professionals), contribute to the smooth operation of clinical activities, including the Locum Clinic, document interventions in electronic medical records, and actively participate in the promotion of healthy lifestyle habits.

Overview of Responsibilities
• Assess patients’ conditions (vital signs, collection of clinical data) and provide clinical follow-up within the scope of practice;
• Support interventions in collaboration with physicians and nurse practitioners;
• Prepare examination rooms and assist with certain procedures;
• Accurately document interventions and follow-ups in the Electronic Medical Record (EMR);
• Provide patient education regarding health, prevention, and condition management;
• Contribute to the management of results, reports, and clinical follow-ups;
• Participate in welcoming patients and ensuring the smooth flow of activities in a variable-volume environment;
• Collaborate closely with the interdisciplinary team to ensure continuity and quality of care;
• Apply infection prevention protocols and ensure patient safety;
• Contribute to the efficient operation of clinical activities at the Nord-Aski Family Health Team and the Locum Clinic.

Required Qualifications
• Diploma from approved Practical Nurse program;
• Current registration and membership in good standing as a Registered Practical Nurse with the College of Nurses of Ontario;
• Experience working in a medical clinic setting or with the use of Electronic Medical Record (EMR) software.

Benefits and Working Conditions
• Salary: According to the current salary scale – $56,189 to $60,782.
• Working Conditions: The Nord-Aski Family Health Team offers a stimulating work environment, including a comprehensive benefits package, participation in the Healthcare of Ontario Pension Plan (HOOPP), as well as opportunities for professional development and continuing education.
• Schedule: Generally, Monday to Friday, between 7:30 a.m. and 4:00 p.m., including one evening shift every two weeks. The schedule may vary depending on the needs of the Locum Clinic.
• Work Locations: Facilities of the Nord-Aski Family Health Team and the Notre-Dame Hospital Locum Clinic.
• Start Date: September 28, 2026
